Enrollment Information
Dear Parent/Guardian:
If you are planning to enroll your Kindergarten, 1st, or 2nd grader here at South Bendle, you will need the following information.
1) Original Birth Certificate, not hospital copy,
2) A current up to date shot record,
3) Your child's social security number,
4) Proof of Residency within the district, do not include your drivers
license, we will accept a utility bill in your name, as well as a copy of your
rental agreement or purchase agreement.
We will provide you with the following information which must be completed:
1) A standard registration form
2) Emergency Procedure Card
3) Release for records or information if your child receives special
education services. This information will provide the basis for a
temporary placement or service.
When completing information for the school, it is important to list accurate phone numbers and adult contacts in the event we cannot reach a parent. The school must be informed of any changes as you make them.
If there are any legal custody arrangements the school needs to be aware of, or special circumstances concerning parental rights, please bring necessary copies, and inform the office as to the circumstances.
Upon completion of enrollment, you should receive a "Code of Conduct" booklet, and lunch menu. We also try to include any of the recent district publications including the Bendle Bugle, Community Education notices, as well as school letters. Extra copies are located on the South wall across from the office area.
Please inquire about paperwork for our "free & reduced" lunch program. Regular price for lunch is $1.30, milk only costs $.30
Our breakfast program begins at 7:45 a.m. The cost is $1.00, milk is only $.30.
